Information you need to know!
Listed below are documents that we would appreciate you filling out prior to boarding your cat(s). You will need Acrobat Reader to view these and to be able to use the fill-in feature.
Everyone should fill out the Purr-sonality form. If your cat has a medical condition, please see the forms in second box and select as needed.
If you choose, you can also print out the form(s) and write in the information. Make sure you bring in the forms with you when you are dropping off your cat(s). We will review them with you at that time, and ask for your signature as confirmation.
Purr-sonality Profile
We know that every cat is different, yet similar. Some of them love to be loved, others would rather be left alone (although eventually they might ask for some love). Some of our cats can play endlessly with a favorite toy, and then snooze until it’s time for dinner! Every cat has their own unique purr-sonality, and any information you can give us will help your kitty acclimate quicker!
Medical Condition Information
Even in the best of times, it can be hard leaving your cat behind when you travel. It’s even more difficult when your kitty has a medical condition. We understand and want you to know your cat is in good hands at Cozy Kitties Inn. Below are some documents that will help you tell us what your cat needs.
